- The following form is to be used for library-to-library transactions only. Individuals are asked not to use the following form but should submit any request to a library with whom they have current membership. Please ask your library about copyright regulations when submitting your request
- Each request needs to be on a separate form. If you have more than one request there is no need to type your personal details each time. Once you have sent your first request, click on the Request Another Item button, leave your personal details and enter new request details
- Use the Interlibrary Document Request Form for journal articles, sections of books or conference papers
